About the Community Circle of Care

Community Circle of Care is a collaboration among the Iowa Department of Human Services, Iowa Child Health Specialty Clinics, the Center for Disabilities and Development and other community partners.

WrapAround Services for Iowa Families

Community Circle of Care is a team approach to supporting and serving children and youth with serious emotional and behavioral challenges. Community Circle of Care serves children/youth birth through age 21 who live in the counties of Allamakee, Buchanan, Clayton, Clinton, Delaware, Dubuque, Fayette, Howard, Jackson, and Winneshiek.

Community Circle of Care focuses on the strengths of you, your family and your community. Every child/ youth served will be surrounded by a support team that is made up of individuals who are important to you. These individuals are identified by you and can include friends, neighbors, pastors, as well as professionals who are assisting in the development of your care plan.

How is Community Circle of Care Different?

  • It is based on the strengths of you, your family and your community. The family’s needs drive how the system responds.
  • It is a team effort with your child/youth, family, professionals, community and supports working together.
  • It is a process where every team member decides to never give up, and provides the care, support and commitment necessary.
  • It is a process that respects who you are. Teams are sensitive and responsive to your family’s culture, language and community.
  • It values the importance of your social supports, your faith community and the community where you live.

Together, the team finds out what is needed to help your child/youth be successful. Together the team finds creative solutions based on your family strengths, needs and culture.

A care coordinator helps you by making sure your family voice is heard and that you are happy with the decisions that are made. They will also assist in creating a wraparound plan. A wrap around plan is driven by the family, is guided by the youth, and is community based. A wrap around plan is based upon your family's strengths and needs that are developed in coordination with you during a Circle of Care meeting. The Care Coordinator will help you revise the plan if it is not working or if your needs change.The Care Coordinator will also assist you in developing a crisis intervention plan that meets your needs.
